Does UM9003F is a chip name on the card?
If so, the card has name E2003U-3.1.

The JP1 jumper block has six jumpers labeled
ROM, MEM and I/O respectively on the card which
are used to select Base I/O adress, Boot ROM adress
and ROM enabling function.
Open first four jumpers in you do not use remoute
Boot ROM.
Last two jumpers setup I/O adress for your card.
Use hte next table to setup the correct adress:

I/O1 I/O2 Base I/O adress
open open 300H (default)
open short 320H
short open 340H
short short 360H

Jumper JP3 must be in A position if your PC is
fully IBM AT/XT compatible. If you are not sure or
card doesnot work properly, try set jumper in B position.
