Hi, I was stupid enough to borrow one of these cards of a friend. After playing around with the jumpers for an afternoon, i managed to get a post with a HDD, CD-ROM and an FDD. This is what I advise:
locate the set of 6 pins and 2 jumpers to the left of the main UMC chip. Set the jumpers 2 jumpers to the most westerly position.

Like so, if you can imagine the brackets as jumpers. I take no responcibility if this goes pear shaped!
